First day of school questionnaires!
At the beginning of every school year I like to have each of my children fill out these first day of school questionnaires. I love to see how their answers change through the years.
My kiddos love to fill out these questionnaires!
I’ve made two different questionnaires, I like to use one of them more specifically for children who can write well. The other, more simple version, can be used by younger children still learning to write. Although, my older kiddos still love to fill out the more simple version too!

What our “first day of homeschool” looks like
I like to make the first day of school a little extra special and fun! These worksheets help do that! Typically we start the day with these worksheets, move into a LA and Math lesson and then we take some fun first day of school pictures! Usually the weather here in Northern Nevada is still warm by the time we start school, so the kids spend much of the rest of the day doing something fun outside. We have a blow-up bounce house and slide that I like to set up, this really just serves as extra confirmation that we’re having a great first day of homeschool and kicks our school year off on a positive note! You could do a park day, pool day, etc. instead.
*Remember that these serve a purpose of the child exploring their answers and building self confidence in themselves! They are also a great sentimental keepsake. If your child fills out a question in a way that bothers you, try to let it go or simply redirect. For example my oldest used to put “Taylor Swift” for her favorite song. Obviously Taylor Swift is an artist not the name of the song. Try to redirect the child, “Oh! You like Taylor Swift! That’s the singer, do you know what song she sings that you like?”
“About Me” – Simple Version – First Day of School Questionnaire
This worksheet is great for toddler’s preschoolers, and older! Even my 1st and 3rd grader still love doing this one!
If your child isn’t yet able to write their own name, age, etc. you could fill that in for them, or you could use a highlighter and write it in and then have them trace it.
For the empty “favorite color” box I like to just have the child fill in the box using a colored pencil. It can be fun, (for us and them), to lay the colors out in front of young children and see which one they choose. One of my daughters always chooses to do rainbow!
My 4 year old was actually able to trace her hand completely by herself this year! I was surprised as this wasn’t the case for her older siblings at that age. Don’t be afraid to give your child a hand with this if they need a little help.
